Life Teen Youth Ministry rEGISTRATION
Fee Per Student - 6th to 12th Grade: $30
Middle school and high school registration fees help cover the cost of Life Teen materials, program supplies, meals and other expenses associated with providing a meaningful and engaging youth ministry experience at COR.

2. Annual Medical Information Form E-Sign (one per child)
3. Media Release Form E-Sign - list all names on one form
4. Parent Permission for Direct Contact with Minors E-Sign (6th-12th grades only)
5. Authorization for Transportation of Youth Ministry Student
